YMCA After-School Karate Program Registration Forms:
Please fill out one form per student you wish to enroll.
Once Southeast Karate has received your registration form, we will contact you with a welcome email with additional information. You will also receive an electronic invoice for your child(ren)'s participation in the program.
Please note that registration for the program is not complete until Southeast Karate has received payment in full for all students enrolled.
Students must have a uniform for participation and uniforms are available for purchase with registration. Please select the appropriate size based on your child's height and weight according to the sizing chart.
Southeast Karate is not responsible for incorrect sizing. Should you select the wrong size by mistake you may order an additional uniform at your own expense.
